Payment is due 2 weeks prior to surgery. Personal checks are only accepted 2 weeks prior to surgery. There is a 25$ fee for returned checks.

Most of these packages will include all transportation and hospital costs*

Please have your card handy to give your information to the receptionist including:

Co-pays, deductibles and co-insurance. We are not responsible for verifying your information. Co-pays are due on visit. Deductibles are to be paid prior to surgery to Dr Jeneby. We file insurance as a courtesy only and does not absolve you of payment to us. Unpaid amounts will be sent to collections after 2 unsuccessful attempts to collect!
